Financing the Health system;







Health systems in Africa tend to operates as a mixed economy evenly split between private enterprises and public institutions. The delivery of healthcare services is structured along three tiers of government;
Federal, State, and local governments.
The Federal government ministries of Health acts as the central policy maker and financer often with a limited role in proving tertiary level healthcare through direct control of the university teaching hospitals.





In Nigeria, approximately 60 -80% of total government spending on health is channelled through small numbers of teaching hospitals. The Health ministries are responsible state general hospitals and delivery of care at the secondary level. Primary care and local dispensary services are managed at by the local governments under the supervision of the state governments.

There is widespread general neglect and lack of investment in public health services. For exampe in Nigeria, as of 2005, total government spending all three tier levels accounted for only 21% of total health expenditure. 68% of total health expenditure was out of pocket, development partners and firms accounted for 4% and 3% respectively Insurance companies account for 60% of spending from firms, which represents only 2% of total health expenditure,demonstrating a serious gap in financial disaster protection. This suggests a high degree of inefficiancy in the system.

The nigerian Government is now slowly striving for greater improvements towards efficiency and access, and a shift towards the bismarck model of compulsory insurance under the NHIS scheme. However, the challenge remains to deliver higher-quality care at lower cost. There is a need to design a systems which leverages the vast network of private sector providers, and also products that motivate consumers to use health services without abuse. there is a need for a more effective care management pathway and reimbursement systems to which the government is committed and fully accountable for patient outcomes. The convergence of medical technology and innovations in IT, reforms in the financial services sector, improved funding for local pharmaceutical manufacturing and the vast demographic demand for improved healthcare services means the challenge is all together now achievable.
